Live Well is a Direct Primary Care (DPC) practice in Sarasota, Florida, serving patients across Sarasota and Manatee counties. We replaced the rushed, insurance-driven visit with something simpler: a flat monthly membership that gives you unhurried appointments, transparent pricing, and direct access to your own provider. No copays, no surprise bills, and no insurance company deciding how your care should go.
What is Direct Primary Care?
Direct Primary Care is a membership model for primary care. Instead of billing insurance for every visit, you pay your provider a predictable monthly fee, and in return you get the kind of access that traditional practices struggle to offer — longer appointments, same-day or next-day availability, and the ability to reach your provider directly by phone or message. Removing insurance billing from the relationship means your provider answers to you, not to a billing code.
DPC is not health insurance, and it is not meant to replace it. It handles roughly 80% of what brings most people to primary care. Members typically pair their Live Well membership with a high-deductible health plan, a health sharing ministry, or Medicare to cover specialists, hospital stays, and emergencies.

Meet your providers
Your care at Live Well is provided by two experienced nurse practitioners who take the time to know you:
- Rachel Martinez Franzen, MSN, APRN, FNP-C — a board-certified Family Nurse Practitioner caring for patients across the lifespan.
- Julie Dooley, MSN, APRN, AGPC-C — a board-certified Adult-Gerontology Primary Care Nurse Practitioner focused on adult and older-adult medicine.
Both believe the best health outcomes come from a real, ongoing partnership between you and your provider — which is exactly what the DPC model is designed to protect.
