In Concierge Medicine, patients pay an annual retainer fee for more personalized and on call care, while DPC involves a monthly or annual fee for access to primary care services without copays.

With a membership to Live Well DPC co-pays for office visits, acute visits, telephone or virtual visits are eliminated. Many DPC patients pair with their membership a high deductible, lower premium insurance plan or a health care sharing ministry plan for coverage in case of a major health issue requiring hospitalization or specialists care..

Q: What is a Nurse Practitioner?

A nurse practitioner (NP) is a nurse who has advanced clinical education and training. NPs share many of the same duties as doctors. They perform physical exams and provide treatment to patients including but not limited to ordering, performing, and interpreting diagnostic and laboratory tests; making diagnoses, prescribing medication and non-pharmacologic treatments; coordinating care; counseling; and educating patients, their families, and their communities. A nurse practitioner must have a graduate-level degree of education. Nurse practitioners focus on health promotion, disease prevention, health education, and counseling. Q: Do you bill insurance?

We do not bill or contract with any insurance plans except traditional medicare (no replacement plans accepted). In some cases, you may be able to submit a receipt for your membership fees/labs or medication cost. Some insurance companies may apply these payments toward a deductible however, you must verify this with your individual insurance.

Q: Do I get discount on labs and imaging?

Yes! Live Well DPC partners with local labs/and radiology facilities to give our patients the very best prices for labs and imaging. We even provide some medications within our office at a low cost saving you a trip to the pharmacy when you aren’t feeling well.

Q: Do I need health insurance?

It is important to carry some type of insurance for life's unforeseen medical emergencies. High deductible plans make an excellent companion to Live Well DPC as due health care sharing ministry plans. In addition to traditional insurance, there are faith based and non-faith based voluntary healthcare cost sharing ministries that are acceptable options to meet the individual mandate for health care coverage in the affordable care act.

Q: Can I cancel or restart my membership?

We believe that building a long-term relationship with our members is the key to optimizing their health. Therefore, please note that a request to restart a membership is subject to practice approval and availability. If accepted a re-enrollment fee of $100 will apply.
